Any appointment missed, late cancelled, or changed without 24 hour notice will result in a charge of $50

bull Last years income tax if you are a new client
bull Identification
bullSocial Security card or ITIN (for yourself, spouse and kids)
bullBirth Certificate for dependents
bullSchool Record or Daycare letter for each dependent
bull Dependent Provider, Name, Address, Tax ID and S.S.N.
bull Banking information if Direct Deposit Required
